Walden Place: Residential Program

The following information is important for you as you are admitted to the Residential Program

Necessary Items
Health Insurance Card
License/Identification
A list of current providers and their numbers
Please have money available for medication co-payments, cab fares (for emergencies and discharge planning) and personal items.

Toiletries
Items to consider, but not limited to:
Feminine Products
Shampoo and Conditioner
Soap
Lotion
Hair Products
Toothbrush and Toothpaste
Cotton Balls and Q-Tips
Additional items may be purchased while in residential care.

Optional Items
Cell Phone without Camera/Internet Capabilities
Hobby or Special Interest Items, such as craft materials, movies (not rated R), books, crossword puzzles, Sudoku, favorite music, knitting or crocheting materials
Photos of Family and Friends
Recipes for Meal Planning
Journal
Paper or Folders to use in groups

Medications
Bring prescription medications only. Please do not bring any open bottles of vitamins or over-the-counter medications. Residents will be expected to purchase their own vitamins and over-the-counter medications, and to make co-payments for medication while in the program. Please bring a list of your current medications, including over-the-counter medications with you.

Clothing
Seasonally appropriate (jacket, gloves, etc.), comfortable clothes (sweatpants, T-shirts), jeans or any casual clothing (to wear on outings). Please do not bring tank tops, short shorts or skirts, high heels or overly revealing clothing. Shoes should be comfortable for walks and community outings. Please try to limit yourself to one bag. Items will be searched upon admission.

Personal items are brought at the owner's risk. Walden Behavioral Care is not responsible for stolen or misplaced items, such as, but not limited to: portable DVD players, iPods, CD players, PDAs, cell phones or jewelry.
